What happens during my first appointment?
On your first visit, we ask that you arrive 20-30 minutes before your scheduled appointment time to fill out new patient paperwork and complete your intake.
The evaluation appointment is the time for your therapist to sit down with you, review your medical history, evaluate your problem areas, and set up a plan of care for your physical therapy.
What do I need to bring to my first appointment?
On your first visit, we ask that you arrive 20-30 minutes before your scheduled appointment time to complete new patient paperwork and complete your intake. If you are being seen for Men's or Women's Health related issues, please be sure to complete our Pelvic Floor section of the paperwork. Patients that are seeing Naomi Watson for the first time will need to complete the additional Intake Form linked below. We will need your insurance cards, physician's order for physical therapy and medication list.

What should I wear to my first appointment?
Please be sure to wear comfortable clothing in which you can move freely or bring a change of clothing for your session. You should also wear flat or athletic shoes. Please do not wear heels.
What if I can't make it to my appointment?
Your therapist has an hour set aside to meet with you on your scheduled appointment date and time. If for some reason you are not able to make that appointment, please call us at 731-300-4950 at least 48 hours prior to your appointment. This will allow us to call someone else who is on our waiting list.
Will my insurance cover physical therapy? What does it pay?
We will make every effort to determine your insurance benefits by the day of your appointment if we have your insurance card prior to the appointment, however, it is important that you contact your health insurance directly to determine your therapy coverage including copayment, deductible, patient responsibility, visit limitation or prior authorization requirements.
